Continuity of Care for Home Care Patients During COVID-19

Home care has become increasingly important as healthcare professionals and families look for ways to assist older loved ones and those with chronic conditions at this time. Our goal remains the same: providing compassionate care from the heart. To ensure the continuity of care for our clients and the safety of our caregivers, we take every update about COVID-19 to heart.

This is a high anxiety time for many families, and we understand that working together is incredibly important to serve our communities. Below are some of the steps we are taking along with recommendations families can take at this time to protect their health.

Health and Safety Precautions

  • Staying Up-to-Date: We are monitoring the alerts issued by the Center for Disease Control (CDC), the National Institute of Health (NIH), and Medicare (CMS) to help current clients and new to receive in-home care safely.
  • Health Screenings: We check-in with our caregivers daily about their health and continue to monitor all contacts and travel. Any caregiver that displays symptoms such as a fever, chills, or coughing is directed to contact their physician and will not be assigned to patients.
  • Protective Equipment: All caregivers wear masks and gloves while visiting clients. We are doing everything possible to source protective equipment for our staff to ensure everyone’s safety while continuing care.
  • Staying in Touch: Communication is more vital than ever now. If you have any concerns about the continuity of care, please contact us. We believe in being fully transparent and can address individual concerns.
  • Limiting In-Person Visitations: To our clients, we recommend that you ask any family or non-family visitors to let you know of their travels and contacts for the past 15 days and health before visiting you. If they are displaying symptoms or have been traveling, please maintain social distance and use telecommunications to stay in touch with loved ones.

At this time, we are working to assist current clients and are accepting new clients to help families care for loved ones in the comfort of their homes. If you are interested in starting home care services or have questions about our patient care, please contact us.

Dedicated to Improving Care at Home Through RPM

Accessible RPM device

Recently, we launched the Accessible Remote Patient Monitoring program to improve patient care outside of hospitals and assisted living facilities. We are the first company with home health care experience providing remote patient monitoring (RPM) technology and services. Our RPM program helps clinicians gain insight into their patient’s health remotely to improve outcomes and avoid hospitalizations while giving peace of mind to clients and family members.

Our HIPAA-compliant system powered by HealthView can support patients with a variety of chronic ailments including diabetes, hypertension, CHF, COPD, respiratory illness, and weight management.  We’re a full-service provider and can supply telehealth devices to qualified patients free of charge.

Our RPM tools take vital sign measurements to ensure care is being provided and can alert medical assistance before problems arise. Accesible’s devices can measure blood pressure, blood glucose, B-Ketone, weight, pulse, SpO2, and temperature.

To get started, clients just need to fill out a prescription for remote care and send it to us. Interested patients can also ask their care provider if RPM is right for them. Once you are on board, our team of nurses will be with you every step of the way. Learn more about RPM.
