Disability Placard and License Plates

The person must submit an MV-9D Disabled Person’s Parking affidavit signed by a licensed doctor of medicine, osteopathic medicine, a podiatrist, an optometrist, or a licensed chiropractor. One must complete the MV-1 Tag and Title Application form to apply for a disabled license plate.
